慢性腰痛とは、腰部に発生する、3ヶ月以上続く痛みのことです。
最も多い原因は、椎間板と後方関節の劣化です。その他の原因としては、強い衝撃や打撃などによる外傷、骨粗しょう症、腫瘍、および感染症などがあります。
主な症状は、腰部に絶え間なく痛みがあることです。これは、局部的な痛みの場合もあれば、鼠径部、臀部、もしくは大腿部にまで及ぶ場合もあります。また、感覚の変化や歩行困難 を伴うこともあります。
診断は臨床所見により、X線、磁気共鳴、もしくはスキャン などの画像検査を用いて行われます。
治療では、鎮痛剤および/または抗炎症剤を用いて痛みを軽減させることを試み、またオピオイドで補う場合もあります。局所コルチコステロイド注射が行われることもあります。絶対安静は推奨されません。むしろ、徐々に体を動かしていくことが奨励されます。理学療法が有益である場合があります。症状の全く無効である場合は、椎間板の神経根を減圧する手術が必要となることがあります。この手術は一般的に良い結果をもたらし、腰部の動きにほとんど制限を残さず、多くの患者が習慣的な活動を再開することができます。
