網膜は眼球の奥にある組織の層です。剥離とは、通常は裂孔(れつこう)により、網膜が本来の位置から剥がれることを指します(牽引や体液の蓄積により剥離が起こることもあります)。
40歳以上の人に多く、女性よりも男性、アフリカ系アメリカ人よりも白人の方に多く見られます。その他のリスク要因としては、網膜剥離の家族歴、近視(マイオピア)、目の外傷、以前に白内障手術を受けたこと、フルオロキノロン系などの薬物の使用などがあります。
初期症状としては、暗いものや不規則な「浮遊物」(飛蚊症)が見えたり、光が閃光のように見えたり(光視症)、視界がぼやけたりすることがあります。病気が進行すると、患者は視界に「カーテン」や「ベール」、あるいは灰色がかった色調が見えることに気づくことがよくあります。この欠陥は夜間には改善し、日中(特に運動時)には悪化します。また、突然視力を失うこともあります。
診断は、検眼鏡検査と超音波検査に基づいて行われます。
眼科医への緊急相談が必要です。治療法としては、レーザー光凝固療法、冷凍網膜固定術、空気網膜固定術、強膜バックル、硝子体切除術などがあります。無症状の患者や、リスクの高い特徴(視野欠損、視力低下、硝子体出血、細隙灯顕微鏡検査での硝子体色素)を持たない患者は、初期治療を必要としない場合もありますが、引き続き慎重に経過観察する必要があります。
